1/ Cơn đau bắt đầu lúc nào? Ngày bắt đầu? giờ bắt đầu?

-When did the pain start? At what date? At what time?

-Quand la douleur a –t-elle commencé? Heure de début? Date de début?

2/ Cơn đau khởi phát t ừ từ hay đột ngột?

-Did pain begin little by little or suddenly?

-La douleur a –t –elle commencé progressivement ou brusquement?

5/ Cơn đau có làm anh thức giấc đêm khuya không?

-Did the pain wake you up  at night?

-La douleur vous réveille –t-elle la nuit?

6/ Cơn đau có làm anh khó ngủ hay không?

-Does it make you can’t sleep well?

-Vous empêche –t-elle de dormir?

7/ Cơn đau có làm anh phải nằm yên không cử động không?

-Does it force you to lie  down

-Vous oblige-t-elle à rester allongé sans bouger?

8/ Cơn đau có bắt anh phải nằm ở tư thế nào để giảm đau hay không?

-Does the pain force you to rool around seeking a comfortable position?

1/ Cơn đau liên tục hay từng cơn?

- Is it a continous or colic pain?

- Est-ce que une douleur continue ou intermittente?

2/  Anh có cảm giác đau giống như dau đâm hay bị bóp (siết)  thắt lại không?

-Is it a sharp pain or constricting?

-Avez-vous l’impression d’un serrement?

3/  cơn đau có di chuyển đi nơi khác không?

-Does the pain displace?

-Est-ce que la douleur a-t-elle changé d’endroit?

1.5. Sự tiến triển của cơn đau, Progression of the pain, Évolution de la douleur

1/ Cơn đau có tăng dần hay không?

-Does the pain increase steadily?

-Est-elle augmente  progressivement/ graduellement/ lentement ?

2/ Nếu đột ngột xuất hiện ở múc độ cao nhất, sau đó cơn đau có giảm không?

-If the pain began at peat, did it decrease?

-Si elle a débuté d’degré maximal, est-ce que a-t-elle diminué graduellement?

3/ Cơn đau có xuất hiện từng cơn hay không?

-Does the pain fluctuate  (comes, goes and come again) ?

-Est-elle fluctuante (elle vient, disparaît et recommence)?

4/ Cơn đau có lan rộng không? Nếu có thì có lan  lên trên hay xuống dưới, phải hay trái? Ra phía trước?

-Does the pain spread? If  yes, it goes to upper or lower, right or the left side? In the front?

-Ese-ce que la douleur irradie? Si oui, elle vient à la région supérieur ou inférieur ; la droite ou  la gauche? en avant?

 1.6. Sự kéo dài của cơn đau, Duration of the pain, Durée de la douleur

1/ Anh bị đau như thế bao lâu rồi? mấy ngày? Mấy tháng? mấy năm?

-How long have you had this pain?  Days? Months? Years?

-Depuis combien de temps avez vous cette douleur? Jours? Mois? Années?

2/Mỗi cơn đau kéo dài bao lâu?

- How long does the pain go when you get it?

-Combien de temps dure-t-elle quand vous l’avez?

6/  Cơn đau có tự hết hay không? Hay cần sự chữa trị của anh hay của bác sĩ?

- Did the pain end spontaneously? Or as the result of  treatments by yourself or by a doctor?

-La douleur a-t-elle cédé spontanément?

7/ Cơn đau chấm dứt một cách đột ngột hay từ từ?

-Did the pain end suddenly or gradually?

-A-t-elle cédé brusquement ou graduellement?

1.7. Các yếu tố làm tăng và giảm bớt, Exacerbating and Relieving Factors, Facteurs  Aggravants et Calmants

1/Có điều gì làm khởi phát cơn đau và làm nó trầm trọng thêm hay không?

-What brings it on and what makes it worse?

-Qu’est-ce qui entraine cette douleur et l’aggrave?

2/ Cơn đau có giảm với thuốc giảm đau hay không?

-Does the pain decrease with medicaments?

1/ Cơn đau có kèm theo buồn nôn hay không?

-Is the pain associated with nausea?

2/ Cơn đau có kèm theo nôn ói hay không?

-Is the pain associated with vomiting?

3/ Đau có kèm theo sốt không?

4/ Anh có cảm giác sưng phù hai mí mắt, mặt, mắt cá chân hay toàn thân không?

-Do you feel any swelling under the eyes, around your face, around the ankles or all body?

- Avez-vous l’impression d’avoir enflé au-dessous des yeux, au niveau visage, au niveau des chevilles ou de tout le corps?

5/ Từ lúc bị bệnh tới giờ, anh có lên cân không?

-Did you gain any weight since your illness?

-Votre poids a-t-il augmenté depuis que  vous êtes malade?

1/ Anh đi tiểu như thế nào? Bình thường? nhiều? ít? Không tiểu?

- How is your  urine  passed? Normal amount? A lot? Little amount? Not at all?

-Urinez-vous normalement? Abondamment? Peu? Pas du tout?

2/  Nước tiểu của anh trong, đục hay có máu không?

-Is your urine clear,  cloudy or bloody water?

-Les urines sont-elles claires, troubles ou sanglante?

3/ Anh có tiểu ra sỏi hay không?

-Have you ever passed stone in the urine?

-Avez-vous rejeté une pierre?

1/  Việc đi tiểu của anh có khó khăn gì hay không?

-Do you have any trouble passing water?

2/ Mỗi ngày anh đi tiểu bao nhiêu lần?

-How often do you urinate during a day?

-Combien de fois urinez-vous par jour?

3/ Ông có phải thức giấc ban đêm để đi tiểu hay không?

- Do you wake up at night to pass urine?

-Êtes-vous réveillé la nuit parceque miction?

4/ Lượng nước tiểu mỗi lần đi? Bình thường,  nhiều. hay  ít?

- For each time you go to the toilet, how much urine do you pass? A normal , a huge or minimum quatity?

-Quelle quatité urinez-vous à chaque fois?miction normale, abondante ou minime ?

5/ Số lượng nước tiểu trong một ngày?

-How much urine do you pass every day?

-Quelle quantité urinez-vous pas 24 heures (en litres)

6/  Anh có bị đau khi tiểu hay không? Đau trước khi tiểu, trong khi tiểu hay đau sau khi tiểu

-Do you get any pain when you pass urine? Is the pain begin before , during or after micturition?

- Avez-vous des douleurs pour uriner? Ces douleurs commencent avant, pendant ou après la miction?

7/ Anh có đau và mắc tiểu nhưng tiểu không được hay không?

- Do you have any pain and unsatisfied desire to pass urine?

-Avez-vous des douleurs et des faux mictions?

8/ khi đi tiểu anh có phải gắng sức hay không? Nước tiểu ra mạnh hay yếu?

-Do you have effort to pass urine? It is a good or a weak stream?

-Avez-vous besoin de pousser pour uriner? C’est bonne ou faible?

9/ Anh có tiểu không tự chủ không?                                        

-Do you suffer from urine incontinence?

-Souffrez-vous d’incontinence urinaire?

10/ Anh có từng bị bí tiểu chưa?

-Were you ever unable to pass urine before?

-Avez-vous déjà souffert d’une impossibilité d’uriner?

11/ Nước tiểu của anh có màu sắc như thế nào? Trong suốt, đậm, đục hay màu đỏ?

-What is the colour of your urine? Clear, dark, cloudy or red?

-Quelle est la couleur de vos urines? Claire, foncée, trouble ou rouge?

12/ Mùi của nước tiểu ra sao, bình thường hay khó chịu hơn?

-What is the smell of your urine, normal or offensive smell?

-Quelle est l’odeur de vos urines, normale ou nauséabonde?

13/  Anh có tiểu ra máu không? Nếu có, khi nào, bao nhiêu lần?

-Have you ever passed blood in the urine? If  yes, when and how many times?

-Avez-vous uriné du sang? Si oui quand et combien de fois?

14/ Máu xuất hiện lúc nào, mới tiểu, giữa dòng, cuối dòng  hay  từ đầu đến cuối dòng?

- Does the blood appear at the beginning, in the midle, at the end or during all the micturition?

-Le sang apparait-il au début, au milieu, à la fin ou toute la miction.

15/ Anh có uống thuốc gì trước khi tiểu ra màu đỏ hay anh có ăn củ  không ?

-Do you take medicine or eat sugar beet?

-Avex-vous boire médicaments ou manger betterave?

16/ chị có đang trong thời gian hành kinh hay không?

-Do you have menstrual period now?

-Avez-vous vos reglès maintenant?
